CYO Physical Night to be Held May 24

A Physical Exam is required annually for each child participating in CYO sports (Grades 3-8).  We encourage you to take advantage of this as it can be a convenient way to meet this requirement for any CYO sport that your child will be participating in for the 2018-19 school year (Volleyball, Cross Country, Football, Basketball, Cheer, Track & Field).  A copy of the form will be scanned and kept on file so that there are no hassles in getting this information to the respective coaches prior to each season.  Getting this done on May 24^th will avoid any need to scan and submit the form at a later date.

The physical is good for one year - your athletes will be set for all upcoming 2018-19 CYO sports.

Please bring the attached form with you and fill out all necessary sections before you arrive.  Cost is $10.00.  Checks can be made out to St. Mary Boosters.
